How many starships named Bradbury have appeared in Star Trek? (Note: Movies and television only.)

According to the Memory Alpha wiki the first U.S.S. Bradbury was mentioned in the TNG episode “Ménage à Troi.”
In 2366, Bradbury ferried Starfleet Academy students from Betazed to Earth. Ensign Wesley Crusher of the starship USS Enterprise-D was scheduled to be transported by the Bradbury to the Academy to begin his classes. Because of an emergency aboard Enterprise-D, the Bradbury was forced to leave him behind to keep its schedule.
A second “alternate reality” U.S.S. Bradbury is referenced by Star Trek: Into Darkness:
The USS Bradbury was a Federation starship that was in service to Starfleet in the mid-23rd century. In 2259, Spock was transferred to the USS Bradbury under Captain Frank Abbott. (Star Trek Into Darkness).
